By accessing and using the services of EVA-LIVING Support Services Inc. (“EVA-LIVING,” “we,” “our,” “us”), you agree to the following Terms of Service. Please read carefully.
Scope of Services
EVA-LIVING provides home care, community support, and healthcare staffing services across Ontario. Services include, but are not limited to: Personal Support Workers (PSWs), Developmental Support Workers (DSWs), Registered Practical Nurses (RPNs), companionship, residential support, and social service work.
Client Responsibilities
Clients agree to:
- Communicate scheduling changes or cancellations in a timely manner.
- Provide accurate and up-to-date information relevant to their care needs.
- Treat caregivers and staff with respect and dignity.
- Ensure a safe environment for staff providing in-home services.
Service Limitations
- EVA-LIVING reserves the right to refuse or discontinue services if staff safety is at risk, if false information has been provided, or if service expectations fall outside our scope of practice.
- Services are subject to staff availability and may vary depending on location.
Privacy & Confidentiality
We are committed to protecting your privacy. All personal and health information is collected, used, and disclosed in accordance with PHIPA and PIPEDA. Please review our Privacy Policy for details.
Accessibility
EVA-LIVING provides services in compliance with the Accessibility for Ontarians with Disabilities Act (AODA). Clients may request services in alternate formats or accommodations where needed.
Fees & Payments
- Service fees will be clearly outlined before commencement of care.
- Payments are due according to the agreed terms at the time of the contract.
- EVA-LIVING reserves the right to update pricing with prior notice.
Cancellations & Rescheduling
- Clients are required to provide at least 24 hours’ notice for cancellations.
- Late cancellations or no-shows may result in applicable fees.
- EVA-LIVING reserves the right to reschedule services in cases of emergency or unforeseen circumstances.
Liability
- EVA-LIVING is not liable for damages or losses outside the scope of agreed services.
- Clients agree to release EVA-LIVING from liability except in cases of proven negligence or misconduct.
Modifications to Terms
EVA-LIVING may update these Terms of Service at any time. Updated terms will be posted on our website with the revised date. Continued use of our services indicates acceptance of the updated terms.
Governing Law
These Terms of Service are governed by the laws of Ontario and applicable federal laws of Canada.
